PRINCE2® Issue Report Template

Here you can find a simple Issue Report template in Microsoft Word format, along with explanation on how to use it.

Background

Wait! You don’t create an Issue Report for each an every issue. It’s used when you want to ask Project Board for advice.

The report mainly a representation of the record in the Issue Register, plus your opinion about possible solutions. Also leave a place for the decision of the board. They will add it to this document, sign it, and return it.

Finally, remember that the Project Board is supposed to be focused on the high-level aspects of the project, and the Executive has appointed you to take care of issues, unless they are out of tolerance. So, it’s fine to ask for their advice sometimes, but you’re not supposed to do it all the time!

Composition

The template has these main sections:

  • Document information: this is the generic information about the document and connections with your Configuration Management system. Feel free to add more information here, but it’s best to keep it simple.
  • Approval: this section is for capturing approvals. There are multiple lines, because you may have multiple versions of this document approved. If you have a well-formed Configuration Management system to track approvals, feel free to remove this section.
  • The Issue: information coming from the Issue Register.
  • Impact: also comes from the Issue Register… probably!
  • Recommendation: that’s where you document the solutions that you can think of, and probably the one that you find best.
  • Decision: this is where the Project Board writes their decision and return it to you.
